NOTE: Only, Hearing Aid and Optical Care (Eye Exam and Glasses) vouchers are available online. If you need vouchers for Durable Medical Equipment/supplies, prescriptions, or emergency dental care in addition to hearing aid and optical vouchers, visit one of our partner agencies. If you apply online, you will not be able to get vouchers in person for 12 months.
Step 1: Register as a Client
Register to be an AccessKC client. You will need a valid email address to complete the online application.
The following are the required documents for eligibility:

Photo Identification for all household members 18 years of age and older.
Adults 18 years of age or older may positively identify themselves using a valid passport, state picture ID, school picture ID card, employer ID, etc.
Services cannot be rendered if photo IDs are not available.

For all household members 18 years of age and older, Social Security card, ITIN card (Individual Taxpayer ID Number), or “Other Document.” For individuals who do not use a SSN/ITIN card, another document is acceptable for eligibility. Acceptable documents include:
Birth Certificate
Consular ID card
Foreign Driver’s license
Foreign Passport
Certification of Homeless status and Residency
I-94 card
Local Public School Enrollment card
Voter registration/National Identification card
Tax Documents

You must provide income documentation for all household members aged 18 or older, regardless of whether they receive income. Household income before taxes (gross income) must be at or below 200% of the current year’s Federal Poverty Level (FPL) guidelines.
Required Documentation:
If a household member has income: Provide proof of income before taxes. Acceptable forms of income include, but are not limited to: salaries and wages, SSI/SSDI, retirement income, cash, investment gains, etc.
If a household member does not have income: Provide a signed declaration stating they receive no income.

Documents that must be from the past 90 days:
Social Security Administration (SSA) Benefit Awards letter
Retirement/Pension documentation
Child Support from Missouri or Kansas
Supplemental Nutrition Assistance Program (SNAP) verification
Unemployment Benefits
State or Federal program documentation (e.g., Homeless Program letter, Department of Corrections release papers), attesting to zero income with appropriate dates and signatures.
Important: Clients reporting no income before taxes or no fixed address will be directed to contact an AccessKC partner agency for further assistance.

Proof of residency in the greater Kansas City area for the past 30 days. Acceptable documents include:
Current Utility Bill
Deed/Rental Agreement
Homeless shelter statement confirming living arrangements
Important: Clients reporting no income before taxes or no fixed address will be directed to contact an AccessKC partner agency for further assistance.

Prescription, Medical Order, or Letter of Medical Necessity from a physician’s office, clinic, or other designated health professional that validates a medical need. Durable Medical Equipment (DME) must have a statement from a medical provider within the past 30 days that validates the medical need. Ostomy supplies can be issued with a medical order OR a receipt showing a prior purchase.
No prescription or medical order is needed for dental care, hearing aids or vision care.

Authorization for Release of Data
As part of the AccessKC registration process, you will be asked to waive your rights under HIPAA so we can communicate with the vendor(s) you select to provide the service(s) you need. The online registration offers a simplified Authorization for Release of Data. To review and execute a more formal authorization and consent for information, click here to access that form. After you print and physically sign the form, email it to Administration@AccessKC.org.
Step 2: Find a Vendor
While you wait for confirmation of your registration, find an optical or hearing aid vendor.
Vendors cannot issue vouchers.
Vouchers are issued only for AccessKC vendors.
Step 3: Request a Voucher
Have your Client ID number, valid email address you registered with, and the name and location (if necessary) of the vendor you wish to visit available.
Frequently Asked Questions (FAQs)

No. AccessKC covers the cost of an eye exam and one pair of glasses every 24 months (2 years).

Lens upgrades require a note of medical necessity. This note must specify a medical condition. As with many health insurance plans, anti-reflective coating (ARC), transitions lenses, or progressive lenses may only be:
• Necessary for and appropriate to the diagnosis, treatment, cure, or relief of a health condition, illness, injury, disease or its symptoms; and/or
• Not solely for cosmetic purposes or for the convenience of the client or the provider.

No. AccessKC does not assist with the cost of contact lenses.

Vouchers are issued only for AccessKC vendors.

At this time, you must request a voucher for dental care at one of our partner agencies.

At this time, you must request a voucher for Durable Medical Equipment at one of our partner agencies.

At this time, you must request a voucher for a prescription at one of our partner agencies.

At this time eligible individuals can only get a voucher for optical care and hearing aids on our website. Vouchers for emergency dental care, Durable Medical Equipment (DME), and prescriptions can be obtained only at one of our partner agencies.

An email address is required to allow for communication about your application and to provide you with the acceptance or denial email for the next steps.

Registering as a client allows us to set you up in the system and check your eligibility. After you are set up in the system, you will receive a Client ID that you will use for requesting a voucher.

Vouchers can only be issued 1 time every year (or every 2 years for optical care- eye glasses & exams).
If you need assistance with more than hearing aids and optical care at this time, you will need to visit a partner agency.
